Bob worked with a leading software specialist to create a PROPRIETARY CRM Platform. The Platform integrates predictive analytics to enhance the level of care while maintaining the highest level of care for Medicare patients with two or more conditions.
Our Chronic Care partner contracts with Doctors, Hospitals, and Clinics to onboard, service, and retain patients with CMM services 24/7/365. They charge a monthly membership fee for SaaS (Software), remote, or embedding staff on-site for our clients. Annual Wellness, Transitional Care, Chronic Care, and Complex Chronic Care plus additional codes generate monthly revenue per member while complying with bi-partisan CCM legislation and raising Risk Assessment Scores. We also conduct Annual Wellness Visits.
Our Chronic Care partner will have coaches onboard current patients and engage them for at least 20 minutes per month while creating a care plan for each patient and serving them throughout the year.
